About Us

Nice to meet you.
My name is Toshitaka Watanabe, director of Higashinari Manyo Home Care Clinic.

I became a doctor and have been working as a cardiovascular surgeon for the past 10 years. I entered the world of home visits by chance, and I was drawn to the profound depth of this work, which requires the best possible judgment within limited medical resources while respecting each individual's outlook on life.

Since 2021, I have worked as the director of Shimada Clinic Clover Clinic, where I have treated 617 patients over the past three years and experienced the end of life for 110 patients. In addition, since 2024, I have been working at the Nakamura Clinic of Yukoukai, where I have gained in-depth knowledge of palliative care for terminal cancer patients at home. I have also utilized my experience in surgery to open a new outpatient clinic for bedsores and have been actively involved in treating intractable bedsores.

The most important thing for me in providing palliative care is to listen carefully.
By listening carefully to what the patient has to say, we are able to understand the patient's current problems and learn their values and way of thinking, which enables us to propose the best possible solutions.

Our most important goal in home visits is to help patients live the life they want.
We aim to be a clinic that listens to our patients' voices and provides the best support possible to help them continue living the life they desire for as long as possible.

Furthermore, to support a patient through home medical care, it is essential to cooperate with many people, including not only the visiting clinic, but also the patient's family, visiting nurses, visiting rehabilitation staff, visiting pharmacists, care managers, helpers, hospital staff, etc. We believe that it is our important role to spread our hospital's philosophy throughout the community, strengthen cooperation with various parties, promote medical education, and improve the quality of home medical care throughout the community.

Origin of the name "Higashinari Manyo"?

The name "Higashinari Manyo Home Care Clinic" embodies our medical philosophy and aspirations.

The reason why "Higashi-nari" was written in hiragana

This is because we want you to experience the kindness and warmth that home visits can bring.
Hiragana has a soft, friendly sound, and reflects our desire to be a clinic that is familiar and approachable to local residents, where they can feel free to come and consult.

The name "Manyo Clinic" is derived from the Manyoshu, Japan's oldest collection of poetry. The word Manyo literally means "many leaves," symbolizing diversity and abundance. Inspired by this, the clinic's name reflects our commitment to supporting people of all ages and at all stages of illness, just as the Manyoshu embraces voices from all walks of life.Just as the "Manyoshu" collected the voices of many people across time and social status, our hospital was named after the Manyoshu, hoping to provide medical care that is sensitive to the thoughts and feelings of each of our diverse patients.

During home visits, we come into contact with a wide variety of patients with different ages, illnesses, living environments, etc. Our mission is to respect each patient's values and outlook on life and provide the best medical care possible for each individual.

In addition, "Manyo" has a calm and gentle sound that evokes the expanse of nature and the interconnectedness of life. Unlike hospitals, home visits allow patients to receive medical care in the most comfortable environment of their own home. We want to support patients and their families so that they can spend their time as calmly and with peace of mind as possible.

Higashinari Manyo Home Care Clinic aims to be a clinic that works with the local community to support people in living their lives in a way that is true to themselves, while respecting diverse values.

The motif of hands embracing a heart symbolizes "medical care that values the patient's feelings and is close to their heart."

The colors of the heart are likened to the changing colors of leaves, with green symbolizing childhood to adolescence and red symbolizing adulthood to old age, representing our hospital's desire to support people of all ages and at all stages of life.

The person in this logo is based on me. I strive every day to be someone who can give people a sense of security.

Home Medical Care (House Call Services)

What is Home Medical Care?
Home medical care is a service in which a physician regularly visits patients at home who have difficulty visiting hospitals or clinics. This care is provided on a routine basis and includes 24/7 emergency support when necessary.

Who Is Eligible?

  • Individuals who find it difficult to visit a hospital or clinic

  • Those who wish to receive care at home or in a care facility

  • Patients who are bedridden or have significantly reduced activities of daily living (ADL)

  • Individuals with cancer or chronic illnesses who prefer home-based care

Medical Services Provided

  • Regular home visits (once or twice a month)

  • 24-hour emergency support

  • Palliative care at home (including pain management)

  • Management of medical devices
    (e.g., gastrostomy tubes, oxygen therapy, ventilators, etc.)

  • Support for family and caregivers (consultation, guidance, and education)
